Upstart Business Journal recently featured PromptWorks co-founder Mike Nicholaides’ thoughts on software quality and responsible software engineers’ obligations to minimize risks.

Washington State officials revealed last December that they had mistakenly released more than 3,200 prison inmates early because of a software glitch.

It gets worse — the malfunction resulted from a software update installed in 2002. It gets even worse — the bug wasn’t even discovered until 2012, but they’re just now getting around to fixing it.
